Outpatient Rehab Services
Southern Oaks Care Center currently offers outpatient rehab services to individuals who are living in the community through our therapy provider, Phoenix Rehab.
Services Include:
• Licensed physical, occupational and speech therapy services 5-7 days/week
• Physical oversight – all services are ordered by a physician
• Individual treatment programs
